New Patient
Welcome to The Cardinal Orthopaedic Institute. In order to make your initial appointment go as smoothly as possible, we are asking that you follow these instructions:
- Please arrive approximately 15 minutes before your appointment time so that we can make sure your paperwork is complete and copy your insurance cards. Print out, complete, and bring with you the following forms. This will not only save you time, but allow you the opportunity to complete them in a more comfortable environment:

Bring with you any X-rays, CT scans, MRI films or bone scans related to your current problem that were completed at another location. If you do not bring these documents with you, it may be necessary to reschedule your appointment.
- Remember your insurance card, any referrals that may be required, as well as any co-payment required by your insurance plan. If you are covered by the Bureau of Workers Compensation (BWC), a third-party payer (automobile insurance or attorney), or are uninsured, please click on the corresponding links to obtain a list of our requirements.
