Scheduling

Thank you for your interest in scheduling an appointment with our practice. We will do our best to make your visit as comfortable as possible.

You may request an appointment in one of two ways-

1. Call the office most convenient to you. When you call, please:

  • Keep in mind that not all services are available at each of our offices, and some of our locations have limited hours. Please click here for a complete listing of our offices, the services each of them offers, the hours of operation, and directions to each location.
  • Have the name of your referring physician and your insurance information available.
  • Let us know if you have had x-rays taken or an MRI/CT scan done related to the condition for which you wish to be treated. We will probably ask that you pick these up (whether films or a CD) and bring them with you to your appointment.

2. On-line request form:

  • PLEASE DO NOT USE THIS FORM IF YOU ARE REQUESTING AN URGENT APPOINTMENT. IF YOU NEED AN APPOINTMENT WITHIN THE NEXT 24 - 48 HOURS, CALL ONE OF OUR OFFICES DIRECTLY.
  • Please click here to open the appointment request form. This form will prompt you for certain information, which will make it easier for us to better meet your needs.
  • This form will be sent to us over a secure link, so rest assured your information cannot be viewed by others. Once we receive your request, we will contact you within 24 hours to schedule your appointment.

Important: If you are a new patient or have not been seen in our practice for over twelve (12) months, please print out and complete each of the forms listed below and bring them with you to your appointment:

The Patient Registration Form,
The Medical History Form (adult) OR
The Medical History Form (pediatric), and
The Consent and Financial Policy Form
